What is Geriatric
Care Management? It is not home healthcare, yet
it fits better in that category than any other.
Professional Geriatric
Care Managers are committed to assisting older
adults to continue living in their homes and
familiar communities independently and safely
for as long as possible.
Many people are
not aware of the choices and resources available
to the older adult who may be experiencing loss
of memory, failing eyesight, hearing, physical
agility, nutritional status, and the ability
to think and reason. Bills may not be paid; personal
hygiene, meals, driving ability may be affected.
Safety becomes questionable.
The Care Manager
assesses needs, and identifies resources to address
the needs and problem areas. The Care Manager
is an advocate for the elder, and acts as a liaison
with family members and other service providers.
Services are monitored for ongoing effectiveness
as needs change.

Inc. offers professional Geriatric Care Management
for adults in the Denver Community. We have 5
nurses and 75 companions. Nurses function as
Care Managers and supervise the companions. Companions
assist with activities of daily living, personal
hygiene, light housekeeping, laundry, prepare
meals and perform errands or transport and accompany
the seniors to appointments, shopping, etc.
